Hide supervised facilities when user doesn't supervise facilities

Hide supervised facility option if user doesn't have any supervised facilities
This is a UI change, but one that I think will improve usability because less stuff on the screen is generally better (always). This logic does follow from the UI Styleguide (for what that is worth).
Note: This mock up goes a step further than what I think Mateusz Kwiatkowski suggested.
Logic
If the current user doesn't have any supervised facilities, and just a home facility, then don't show the option to choose between a supervised facility and a home facility.
