Short description

Heading and title classes need to be cleaned up.

Changes should be made across all finished application tickets and be reflected in the OpenLMIS UI Styleguide

Page headings
Headings that are the first element within a .page element should be treating like this:

Note the bottom border

When implemented these should be H1 elements, but H2 elements or elements with .title should have the same style.

Section headings
Form legends, or headings that are not the first element in .page element ( OLMIS-1149 Done ) but are the first element in a form or section element should be treated like this:

Note the indentation from the left, and the line across the middle of the element (this might be best achieved using the :before pseudo class).

These would be H2, H3, H4, legend and .title elements. An H2 class will be larger that H3 or H4 which are the same size as legend and .title elements. H3, legend, and .title classes should be bolded.
