Column appears in requisition form and persists thru the workflow.

Acceptance Criteria:

• When initiating a requisition (for a program whose requisition template uses this new field), the column gets included on that requisition.

• The UI shows that column in the right order and with the correct label/name.

• Users cannot edit this field on the requisition form. It's always a calculated field.

• The calculation should match the formula in the parent ticket.

• Check that this field persists through authorization and approval steps.
