Facility FTP Settings UI

Description

Add UI page which admin would be able to add/update/delete FTP settings for the given facility.

Acceptance Criteria

UI menu accessible for a user with matching privileges
Form is styled to match the UI-Styleguide

@Nick Reid let's discuss this one as well tomorrow. I don't think it is high priority for 3.0. I want it done eventually but the other things we discussed are more important for now. this is an admin screen so we could put it in the admin epic.

@Nick Reid Yes, a facility can have only one transfer properties (FTP/Local).
