Create and Edit Supervisory Nodes

As an admin I need to create new Supervisory Nodes so that I can support additions to the supervisory hierarchy as required.

Acceptance Criteria

1. In the Supervisory Node view page, the admin can select a new "Create Supervisory Node" button that will bring them to the Create Supervisory Node modal.

2. Admin must enter required fields for a Supervisory Node: Code, Name, and Associated Facility. Code and Name are text fields and must include at least one character, Associated Facility is pre-populated with the list of available facilities to assign.

3. Admin has option to enter in optional Description. This Description field is a text field with no validation to save.

4. Admin can select a Parent Node assignment from a dropdown list (see mockup).

5. There is validation for the Supervisory Nodes that can be selected as a Parent Node. Example:
