User with no approval role should not be able to select Supervised Facilities
Description
As srmanager4, when I go to Initiate Report and Requisition, I should not be able to select "Supervised Facilities" because I do not have any facilities that I supervise.
Expected Behavior: When I hover over Supervised Facilities radio button, I should not be able to select it.
All works. When user does not supervise any facilities, the 'Supervised Facility' radiobutton is not active. If administrator assigns supervision roles to the user, the 'Supervised Facility' radiobutton becomes active.
Tested on Chrome and Firefox.
