Total Losses & Adjustments modal field shows errors after adding loss

After entering an adjustment in an R&R form, the total losses and adjustments modal shows form error for selecting another reason and quantity. But user is able to close the modal without causing any errors. The Add button also repositions to underneath the Reason field once you've added a loss or adjustment.
