Make one program in demo data match Malawi columns

Description

Change the R&R template for a program so that the headers match the Malawi headers (headers in Malawi have less characters). The expectation is that the columns resize in the R&R form correctly.

I tested the ticket, and the current EM requisition template is in line with the one for this program in Malawi. Please only add the changelog.

I added screen with current template for EM program in Malawi. You can use it to verify the ticket.

I think you could only check if template has been changed for EM.

Ideally, we could copy the configuration Malawi uses from their Essential Meds program templates into our demo data: include the shortened names of the columns, the column order, and the specific columns enabled/disabled.
