POST /validReasons returns error

When user select any program and facility type for reason, POST /validReasons returns the error.

Reproduction steps:
1. Go to Administration -> Reasons.
2. Click the "Add Reason" button.
3. Provide name and category.
4. Provide program & facility type and click the "Add" button.
5. Click the "Add New Reason" button.
