Too wide columns in certain tables

Description

When I was executing OLMIS-4356, I noticed that columns are too wide in certain tables. Usually, they are just as wide as the text input and not wider. In certain tables, this is not the case. The issue occurs both on Firefox and Chrome, and in the tables on the following screens:

  • Facility Types (the "Actions" column);

  • Requisition Templates > Template Settings (both columns);

  • Users > Roles > Reports (both columns);

  • Users > Roles > Administration (both columns).
