1233: Add 'convert to order' button to requisition page

Short description: If requisition is in 'Approved' state, then user can view 'convert to order option' (if they have relevant permissions User is presented modal with list of options for warehouses Confirmation modal is presented to user to confirm their choice A notification is shown that the requisition has been converted to an order The current requisition page is refreshed

Pre – conditions:

Step	Action	Expected system response	Comment
1	Log into application. From top menu select Requisitions, next select View. Select Facility Name and Program. Click Search.
2	Click record with Requisition, which APPROVED status.
3	Click Convert To Order button.
4	Again click Convert To Order button.	Should appear confirm modal.
5	Click OK button.	The current requisition page is refreshed
